Advance Care Planning – Preparing for the Future

Advance care planning helps ensure that your wishes are respected if you are no longer able to manage your own affairs because of illness or disability.

Care Directive

A care directive allows you to specify who should be appointed to act on your behalf and which wishes should be taken into account.

Power of Attorney

A power of attorney authorizes a trusted person to make decisions for you in specified areas.

Important: A care directive does not replace a power of attorney. Both instruments complement each other and help preserve your right to self-determination.
